PLEASE BRING ALL INSURANCE CARDS AND A PHOTO IDENTIFICATION TO EVERY APPOINTMENT. ALL CO-PAYMENTS ARE EXPECTED AT TIME OF VISIT.

Cancellations

If you need to cancel an appointment, please notify us at least 48 hours in advance. We will reschedule your visit.

If Your Are A New Patient

Please arrive 30 minutes prior to your scheduled appointment in order to complete your Patient Information Packet. Or download the Patient Information in advance here. You may be asked to leave a urine sample. Please come to your appointment with a full bladder.

If Your Primary Care Physician Has Referred You to UACC.

UACC is committed to working closely with your referring physician and will send your doctor information regarding your visit, test and/or procedure. Please let us know the name of your referring physician, along with the telephone number and address. We will also request the medical records pertaining to your visit from the referring physician. If you have hard copies of your medical records, please bring them to your appointment and we will scan those into your electronic health record at UACC.

On the Day of Your Visit

Please arrive 30 minutes prior to your scheduled appointment time in order to complete necessary documents, including authorizations for treatment, insurance assignments and medical questionnaires. Please have your insurance card(s) with you for verification. We are required to see your insurance card every time you have a scheduled appointment. You will also be required to pay for any applicable co-payments and deductibles at the time of your visit. Please bring a picture id (driver’s license, RMV ID) for identification.

What to Bring

      • A list of questions or topics you may wish to discuss with your physician.
      • A list of the medications you take or the actual containers of the medications, including non-prescription drugs such as vitamins and herbal remedies.
      • You may be asked to leave a urine sample. Please come to your appointment with a full bladder.
